A transition from one care setting to another is a vulnerable time for patients. Many medication errors occur during these transitions. The Community Medication Management (CMM) tool aggregates multiple medication lists into one shared location allowing healthcare professionals to better understand a patients’ medication history and use that information to make a difference.
CMM is a web-based application that allows providers from multiple care settings to impart vital information about their shared patients in a secure environment.
CAMP (Community Aggregated Medication Panel) is the same information automatically retrieved and delivered directly to your EMR or other secure delivery point. Multiple triggers can be utilized, contact us for more details.
CMM has a variety of tools to assist in medication management.
Automated medication feeds are updated daily or in some cases real-time. Those medications are displayed in a comparison chart, which sorts medication by therapeutic class, grouping medications that are identical, similar and unique into an easy to read format, allowing easy identification of errors, potential drug therapy problems, and deprescription opportunities.
Medication data is collected from multiple sources including:
- Primary Care Providers
- Long Term Care
- Specialty Providers
- Home Healthcare
- Fill History
- Adding more feeds as partners choose to participate
